Im looking for a form for child custody transfer from mother to grandparents.?

I need a form for the insurance company that says Myself and my husband have custody of our grandson. So he can be insured thru our policy. Any help?

  1. When you were awarded custody of your Grandson, you have recieved a paper from the courts that gave you custody. If you do not have that, you can go down to the court house, and ask the clerk of courts for a copy of the custody papers. Sometimes there is a fee, but you should always have these papers handy in case other situations occur to show proof that you are the custodial care taker of your Grandchild. I hope this helps!!


  2. There is no insurance company form.

    You need to show the court documents showing that you have legal custody.

    While I was doing dependent audits, we had someone try to turn in a notarized paper signed by the biological mother and the grandparents stating that the grandparents had custody.  It wasn't acceptable - only legal court documents prove custody.

    If you don't have legal custody granted by a court already, then you need to get a lawyer and get the papers drawn up in court.

  3. Insurance companies don't grant custody.  You'll never get this form, from an insurance company.

    You need to get it from a JUDGE.   In COURT.
