I would like to get some psychological,sociologial and philosophical reasons for the making of such a lesson plan.

  1. A learner centred lesson plan is an approach rather than an actual item.  It's more to do with making the activities within any lesson plan centred around the needs of the learners (whichever kind of learners they may be).

    You would need to consider age group, backgrounds, ability levels, interests, special adjustments etc. and create a lesson plan taking these things into account.

    So I don't think you will find a website with a "model of a learner-centred lesson plan" on it, but you will find many sites with suggestions and many different activities and theories if you just search on google.
