  3. You did not specify what country you are from.

    In the US, you would need to provide acceptable proof of the name change (marriage/adoption/divorce decree), and pay the appropriate fee. The US, unfortunately does not amend passports (insert a new page, or stamp with the new information) anymore.

    If you're in the UK, check here http://www.ukdps.co.uk/AWomansRightsUpon... . It appears that you can change your name at anytime. I'm unfamiliar with UK passport laws, so you may want to contact the UK Passport Service.

    Other countries may have different procedures.
