  1. You apply for a US passport in person at the post office. You need:

    -  2 passport photos (available at places like Kinkos and Walgreens).  Get at least 2 extra passport photos for the visa.

    -  A completed application form, which is available at the post office or from:  http://travel.state.gov/passport/get/fir...

    -  An official copy of your birth certificate (not a photocopy)

    -  Proof of identity (ex. driving license)

    -  $100

It takes about 2 months for your passport to arrive.  If you need it sooner, you can pay an extra $60 for expedited service.

Once you have your new passport you can apply for a visa to India.  You can apply by mail:  http://indiavisa.travisaoutsourcing.com/...

You should call your doctor and ask about recommended vaccinations.  I don't think any are actually required for travel to India, but you should be vaccinated against Hepatitis A & B even if you never leave home.

I also highly recommend you get "trip medical insurance" even if your regular health insurance covers you overseas.  Trip medical insurance is cheap and is sold by travel agencies and airlines (ex. www.americanexpress.com).

Hope you have a great trip!

  2. Contact the Indian embassy in Washington D C for a visa.

  3. Passports are currently taking 4 weeks or less to get .  See site below for all application info and to print out the forms to fill out and see the list of documents you need...save time at the post office.  See second site and read under "Entry Requirements" and use the link there to go to the site to get your visa for India.
