  4. I can't speak for the rules from the BCA as Straight can... but it is also possible that the incoming player that takes the cue ball behind the head-string (after the foul from the break) can also shoot, miss (or make a ball) and scratch for another foul.

    Now, the table is still "open", and the incoming player (the breaker) gets ball-in-hand... anywhere... on the table.

    Like I said, I can't speak for BCA... but the Leagues I've played in this situation is part of our rules.

Yes, a scratch is always a foul.  Because the table is open and suits have not yet been determined, the incoming player takes ball in hand behind the headstring.  On any foul where suits are already determined, the incoming player takes ball in hand anywhere on the table.  M.D.-BCA Instructor/Referee.
