In Batman #676 Issue #1 of Batman R.I.P. what happens at the end

i don't get it. is that the Joker with slaughtered Robin, nightwing, and some other dude. i can't find issue 677 but in 678 there all still alive. what happened?

In Batman #676 the scene you are referring to is a fantasy/dream that Joker is having.  The Doctor is doing the Rorschach inkblot test with the Joker.  That's the test where you are shown inkplots and asked what you see.  When Joker looks at the inkplots he sees the death of Robin, Nightwing and Jim Gordon at his hands.
