Is it legal if I give a candidate a form to fill in which includes place of origin, martial status, ID number, and religion?

  2. Of course it is. How else are they to know anything about you.  They can ask pretty much anything with regards to the position of employment.  You don't have to answer if you feel uncomfortable but then again you might not get the job.

Your questions cannot be asked in an interview.  Whether they will claim single or married is a legal question only if you have hired them already - that is, offered them a job which they have accepted.  Then you require the information to process payroll.  However, at no time do you need place of origin or religion.  Not sure what you mean by ID number - Social Insurance Number probably, and that is required for payroll and to insure they are legally able to work in Canada, so again you can ask for that info after they have been hired.

    You can ask if they are legally able to work in Canada, and in some cases, if they are of the legal age to work in a specific province.  (example, if you are hiring someone to work in a bar, you need to know they are able to serve alcohol).  Each province has different rules that can be found on the specific provincial government job site under the Employment Standards division.
