In Lars and the real girl what was wrong with Lars?

i know he was socially inept but he freaked out when the doctor touched him on the neck ( or back, i cant remember), like he was just touched by fire or something

anybody know?

I thought it was a really extreme case of social anxiety disorder.  Like, he had detached from people and reality and was physically pained by reality and feelings that come with interacting with others on an emotional level.  

p.s. It broke my HEART, that movie.... but I loved it.  :)

he was not used to being touched, so to him it felt like it hurt.

The doctor's touch reminded him of the childhood trauma he had: he lacked an affectionate mother.  Thus, any gentle touch reminded him of losing his mother, and was painful to him.
