In terms of port and address ,how is the message delivered from one PC to another via Internet?

I want to setup a Remote Access to my computer .In order to that, i want to understand first how is the signal transferred from remote pc to my computer in terms of ports and addresses.The remote PC is connected to a router and my PC is also connected to a router.Anybody please help me understand how the internat message reaches from one PC to another in terms of addresses and ports .

  2. I will try to explain what i am assuming you are asking, and that is how you can get to a remote computer behind a router from a computer behind a router.

    I will use vnc as an example, first you have to (normally) use port 5900 and you want that to be redirected by a firewall on your external ip address.  You want to port forward port 5900 to your remote computer behind the router, lets say the internal ip address of that machine is 192.168.1.23.  Then from where you are you will also run vnc(the application that is already running on the remote machine you want to get too) you will use the external ip address such as

    68.180.206.184:5900 in your vnc application and it will hit the firewall and redirect it to the machine inside and ask you for a password to enter.
