I believe all of them have been issued.

none, hov lanes are for many people, but there might be a hybrid lane... then all hybrids are welcome...

Toyota Prius, Honda Civic, and Honda Insight.  Although, in CA, they are all out of the stickers which will allow solo drivers to drive in the carpool lane.

Depends on the state.  

Some states also issue only a limited hybrid HOV lane passes each year, so not anyone with that model can use it.
