Interest/Attitude Survey for Elementary Students?

I'm enrolled in a Diagnosis and Remediation Lab at my school and will be tutoring one student in reading. We are being asked to present an example of an interest/attitude survey that we could use to get to know our student better. Does anyone have any examples of a survey I might use that would not only ask about the student's interests, but also their attitudes toward reading and writing?

I guess it depends on the age of the student.  I teach 1st grade and I have a happy face one.  It says how do you feel when someone reads you a book.  (They fill in a happy in between or sad face) How do you feel when you read to someone?  To yourself?  A teacher asks you to read to yourself?  You read an informational (fact) story?  Read before bed?  etc. For an upper elementary or Jr. High student you could ask the students to rate on a scale of 1 to 5.  I just finished my master's in reading education.  You can make up whatever questions you want, but you need to get an idea of what the students likes and dislikes are and how they feel about actual reading.  It is a way to get a look at who your student is.  I had a student who was terrified to read because he felt pressure from his mom. When I promised him I wouldn't tell mom about how he was doing, he was much more relaxed and did a WAY better job.  Its a way to get a look into the life of your student and build a bridge to helping them.  If you need more specific let me know and I can scan mine and email them to you!  Hope this helps!
