The sources revealed that the report was sent to the PCB headquarters in Qaddafi Stadium, Lahore soon after the five-match One Day International series against West Indies, which the tourists had won by 3-2.
"PCB Chairman http://www.senore.com/Cricket/Ijaz-Butt-c64128 asked the manager and coach to send a report on reported incidents where Afridi walked out of a team meeting and was also reluctant to lead the team in the last two one-day internationals because of selection issues with Waqar," disclosed
a source on Friday.

Afridi’s statement prompted the PCB chairman, Ijaz Butt look into the matter personally. Butt sought a report from the team management detailing the row between the team coach and the skipper.
Sources have claimed that Alam, in his report, blamed Afridi for the incident and complained the whole incident had a detrimental effect on the dressing room.
"The manager in his report outlined the details of the incident and differences between Afridi and Waqar and also highlighted that Afridi had not behaved like a captain during the entire episode and was only persuaded to lead and play in the last two one-dayers
by Intikhab and all this drama had a detrimental effect on the team atmosphere," the source said.
The men in green dismally lost the final two ODIs of the five-match series after clinching comprehensive wins in the first three one-dayers.
