Typically, when one writes "Antigua," the reference is to the island in the Caribbean.  When one writes "La Antigua," the reference is to the "old" capital of Guatemala.  Unfortunately, the confusion is that most people shorten Guatemala's former capital to simply "Antigua."  Incidentally, there have been 3 capitals in Guatemala's history: one now called "Ciudad Vieja" from the time of conquistador Pedro de Alvarado, which was destroyed by mudslides; the next formerly Santiago de los Caballeros now known as "La Antigua", which was destroyed by earthquakes; and now Guatemala City or La Ciudad de Guatemala.
