Top Tier gasoline is tested against standards set by automotive manufacturers to be sure the fuel put in their vehicles will not cause long term damage to the engines.  To qualify as a Top Tier gasoline, all grades of fuel must pass the standards, not just premium.

Since you're asking about Arco, I'm guessing you're on the west coast.  If I had my choice of stations to purchase gasoline at, I'd choose Chevron or Shell on the west coast.  That being said, when Arco was on the east coast, I used to use their gasoline without problems.  They abandoned the east coast market a good 20 years ago, so it's hard to say now what their products are like.  They're not currently listed on Top Tier.

Sure.  It comes from the same hole in the ground, and is refined the same way as all the other gasolines.

I use Chevron. My van doesn't run well on Arco.
