McDonald's is not in every country but they are international with roughly 28,000 locations worldwide.  They are roughly 14,000 units in the US.

yes.  i've seen it in england, germany, china, japan, and france.  actually, england is trying to get rid of them because the menu is too caloric and fatty.  when i went two years ago, the only fat people that i saw were tourist.  okay, that's tmi....
