  1. No, middle name isnt maiden, but you can use your maiden name as a middle name just as long as you are clear, that you are not trying to defraud anyone or mislead anyone on your identity

  2. Your middle name is whatever your birth certificate states it is.  For example, John (first) Henry (middle) Smith (last).  Maiden name is for women.  For example.  Prior to getting married your last name is your maiden name (family name).  So, Sally Smith (maiden).  However, when you get married to a guy, usually, the female will take the guys last name.  So, if you marry John Jones, your "married" name would be Sally Jones.  Now, you could have it Sally Smith-Jones if you want to keep your family name.

So, if you want to use your 'family' name and your husbands last name, you would have to have it hyphenated.  so it would look like this First name, Middle name, family name - husband's last name.

  3. It can be but it isn't necessarily so. When I married, my maiden name became my middle name and my husband's surname became my surname.

  4. Middle name is not your maiden name. If you get married, it is not necessary to change your name at all. In fact, your HUSBAND could change his last name to your last name if he wanted to. Rare, but sometimes it happens. You could be married 20 plus years and neither of you guys had a name change. Majority of the time though, the wife takes on the husbands last name and ditches their maiden names in important documents and such. Some married women like to hold on to both names. They can take on their husband's last name, but also keep their maiden name.  You could change your name to whatever you want. Just think about Prince when he changed his name to a symbol. I hope this helped some if any,

  5. u can use your last name first unless u use your middle name last with the exceptions of using first name last or in certain circumstances u can use your first name first and your last name with the mother one in the middle... i hope this helps..
