  1. No, it is an unincorporated territory of the United States.

No.  Although the atoll is part of the Hawaiian Islands, it was specifically excluded from the state of Hawaii.  This is because the United States annexed the uninhabited atoll in 1867 using the Guano Islands Act of 1856.  The rest of Hawaii remained an independent country until Hawaii was annexed by the United States in 1898.  So, Midway Island has been a separate political entity since before Hawaii became part of the United States.

  3. I've read that it was formed by the same volcano that the plate on which Hawaii rests moved over a long time ago.  I don't believe it is actually part of the Hawaiian Islands, however.

  4. No it is not. It is situated near the northwestern end of the Hawaiian Islands archipelago.
