It can be inherited or mutated.  When it pops up in a family with no history of it, it is normally mutated, but when there is a pattern of it in the family, then it is normally inherited.  Any disorder that is seen is normally the product of a mutation, whether or not it happened in a recent generation or had been there for several generations.
