It can break of course.  I couldn't wear tampons until I first had intercourse and then I was still extrememly tight.

Not usually. I can happen, I'm guessing if the girl uses a big size and forces it in wrong. But generally it doesn't. The opening, even with hymen intact, should be big enough for a tampon.
