  1. UK answer.

The mums maiden name is on the birth certificate  and what ever name she is using at the time of registration. The only change as far as I know with the birth certificate took place on 1st April 1969 when the parents birthplace and usual address was included.

  2. My children's birth certificates had my maiden name, even though I was married.

  3. You will always run into differences (ie a certificate from today vs one from the 1920s, one from the US or one from the UK). For the most part, I believe the maiden name is what is REQUIRED.  However.. nothing is automatic.  I am familiar with some very early certificates/ records in Texas, where the index clearly says that mom's name was not even included. Only the father.  Sometimes the child itself was only called  male/female infant.

  4. yes it is put on there when they create it.

  5. yes

  6. I beleive both go on there.

  7. It is for the last 75 years. But before 1930 it wasn't required in most places. Many times the parents were simply listed as "Mr and Mrs Thomas Smith" or "Thomas Smith and Jenny Smith". The other part is that the hospital doesn't ask you to present ID with your birth name on it and when some women fill out the paperwork, they don't give their maiden names. When I worked in Medical Records, it wasn't uncommon for divorced women to keep their ex-husband's last name after the divorce and the baby was given his name even though he wasn't the father. We have to take what women tell us, then forward the completed certificate to the County. Even fake names aren't uncommon. The woman comes in through the ER with no ID. She's in labor, delivers and goes home with no way of knowing if the name she gave was real or not. So it's not ever "no matter what", even today.
