Is an ABN needed for visual field testing if the diagnosis is not medically necessary to warrant the test?

What if a patient wants the test performed but their diagnosis or symptom is not a valid medicare reason to have the test done? We have a form for the patients to sign stating it may not be covered by insurance but it is not an actual ABN as we use the same form for all patients. Is this good enough?

  1. You're probably okay.  I would make them pay for it the day of the test, however, and reimburse them if by some miracle insurance paid you.


  2. We use a form in our office that we typed up on our own with similar info to the ABN, we use that for refractions and any other

    non-covered services, and have yet to have any issues.
