  1. It's not really a "form" of humidity, but simply indicates that the relative humidity of the air near and at the surface is 100%, or saturated.  Fog is simply a cloud that happens to be in contact with the ground.

  3. I'm no meteorologist, but I've been told that fog is the result of humidity and other specific conditions (that I'm not particularly well-versed in).

    So humidity doesn't always form fog, but fog always results from humidity and other circumstances.

If you have fog at the surface your temperature and dew point are about the same.  It means that the air is saturated (wet) and that causes the fog.   The inversion keeps the fog in place til the temperatures at the surface can break the inversion.   However, strictly speaking,  it isn't a form humidity.
