Gotham is the DC universe version of the Dark side of New York.  Just like Superman's Metropolis is the high rise version of New York.

If you were to break it down, you might consider Gotham City the human level of downtown neighborhoods.

While there might be other cities actually named Gotham, they will never be associated with Batman unless you are talking about a Northeastern LARGE community with Gothic features nestled into the architecture.
