Is it Standard Practice for Medical Professionals to Quote Patients their files?

I went to Planned Parenthood to get treated for an STD I received from being sexually assault. I said "He penetrated my v****a with his finger. About one month later, I noticed small red bumps around my v****a."

I just looked at the copy of my file that I requested and under "Clinician Notes" It says "He fingered me and I got lumps" in quotations.

That is all it says in that area. I thought that medical professionals usually don't quote but rather make statements such as "The patient alluded to the fact that he was experiencing intense abnominal pain the past two days." Or do they often make quotes such as "I have had intense aborminal pain the past two days"

I am trying to understand what is standard procedure so I understand why they quoted me inaccurately. They obviously turned what I said into a quicker form to save time, but it appears in my file that I said those exact words, when I did not.

I would love advice! Is it standard to quote patients in their files?

    Depending on the physician, some do quote verbatim what the patient stated, or what reaction the patient gave. There is no standard procedure. It's up to the individual physician.

  4. I do not know about planned parenthood, but I can tell you that many other health professionals in the psych area are under contract with a large number of corporations and governmental units. The prime directive is not to write anything in patient files that is damaging to an employer. They will write up victims as nut cases to make it harder for legal action against a contracted company/governmental unit. They also 'shake down' those without contracts, by getting contracts after the fact. You would expect this in Russia, but not in the good old USA.
