If you know where you want to visit, research the tour prices online before you leave when you have time to weigh your options and pick the best choice. If it's a good price, go ahead and book it.

It depends on where and when you are going. My experience is that you get to Italy first, then pick a city you want to use as your "home" point. From there, with the help of your hotel's concierge, you can book your tours.
