It is very common for men to have s***n in underwear. It is very difficult to squeeze all the s***n out of the urethra of a p***s after an ejaculation; some could even dry on the tip, sealing more s***n within the urethra that could leak out even hours later. Thus the presence of a s***n stain on underwear is not an indication of poor hygiene.

s***n stains are also not indications of any specific kind of sexual activity; they could occur after intercourse, masturbation (which most boys and men do regularly starting from puberty and throughout adulthood, regardless of other sexual activity), or nocturnal emissions.

Whether a s***n deposit leaves a stain depends on what the underwear is made from and how it is washed; even variations in s***n composition between different men and diets can be factors. For instance, underwear made from microfiber is more likely to stain than traditional "tighty whities".
