Is it difficult to deal with accents in the medical transcrption field?

Or is it not so bad? I want a job where I can mostly just type all day. But I don't want it to be online. Like if a dr. office wanted me to work at home that would be fine I just don't trust online scams, etc. Thanx for your help.
