  1. Try brushing more.

  2. Im a dental nurse,  you should use plenty or fluoride toothpaste and also use a fluoride mouthwash.  If you are going to eat sugary foods or drink sugary drinks make sure u do it in one sitting, ie dont sip a can of coke over the space of an hour.  It is not the amount of sugar that is harmful, it is the amount of times in a day your teeth are exposed to it.  Your teeth can deal with 3-4 sugar attacks a day, any more that this and you are at risk of decay

  3. No, its not normal if u don't want halitosis try brushing more and eat less sweets and sugary drinks

  4. Many people get cavities occasionally, but since your teeth need to last throughout your life, you should take better care.  Going to the dentist twice a year is commendable; many people don't bother to do that.  For your own sake, however, you need to brush at least twice a day and floss whenever you feel food between your teeth and at least every-other day.

Getting teeth pulled and getting false teeth etc is painful and expensive.  You'll always be proud of yourself for having all your teeth and no gaps in your smile!
