Is it ok to use Demerol along with a fentanyl patch ?

I have SLE (Systemic Lupus Erythematosus) and am currently in the middle of a bad flare-up. I'm usually given 100 mg. Demerol (Intramuscular injection) for joint pain. My rheumatologist recently put me on DUROGESIC fentanyl patch (50 mcg). I'm now on the 2nd day since application of the patch. So far, the pain has not decreased though. My doctor said that some people require additional medication until the fentanyl enters the bloodstream. Is it safe to take Demerol as well with the patch ?

  1. The only problem in SLE is that any medication introduced will eventually not work at some point and then to ease the pain stronger opiates are given. In your case, I don't like this idea of taking numerous medications on and on if they are not working. Only take those medications that do give you some pain relief as to begin with. Now coming back to fentanyl patches, yes it is true that most people are on other opiate medications prior to using these patches.

    In Your case, You are already using Demerol but seems like that is not working right now. I suggest that You request your doctor to have You discontinue Demerol and prescribe " Codeine " instead. And yes should You feel like continuing with the Demerol and be using those patches at the same time, there is no harm.

Demerol and Fentanyl aren't counter indicated and are therefore ok to use in conjunction.  Demerol (IM) probably isn't your best course for pain control.  IM pain meds usually don't work much better than oral ones, therefore a better option may be Norco (codine with only a little bit of tyelonol due to possibility of kidney problems).  

    The fentanyl will work eventually as it is 100 times more potent than morphine.  You need to be careful to use it only as directed.  

    You also may want to have your Prednisone bumped up or possibly even get a methotrexate pulse.  This will help get the inflammation down as this is what's causing your pain.

  3. Fentanyl and pethidine (demerol) don't interact directly, you should be fine.
