Yes. The phone company receives orders for line adds and disconnects. Yours could have mistakenly been moved to someone elses or theirs to yours. They even could cross connect you so that there are 3 or more on the same line, but thats unusual.

Absolutely.  There are three ways.

1)  You can do this yourself (or someone with access to your phone).  They can activiate call forwarding.

2)  A repair man in the field could get the wires crossed.

3)  A repair man in your local Central Office (within 5 miles of your home - typically), could of gotten some wires crossed.
