I want everything submitted in the boxes to be sent to my email

Gaming, Email, 18 should be Square Check Boxes, meaning Multiple Choice

"Click here to proove you are not a bot:" Should have a Check Box that doesn't allow someone to press the submit button unless it is checked.

The E-Mail sent should look like

"E-Mail Set-Up:

Subject's E-Mail: [e-mail]

Name: Mister

Last Name: Anderson

Password: 1337dude

What kinds of things they wanted to use the e-mail for: Gaming, Email, 18

Bot Check: Passed"

^ That would be an example of someone who wanted an e-mail, passed the bot test, and wanted the e-mail for all 3 reasons
