Is it possible to request a copy of my SSN card application? and how?

What do i do?

Is there another document i could request where it states that they accepted my application and issued a SSN card to me?

Hi, it's rather simple actually, more than you'd think.  Go to the social security site and fill out the form.  :) If you live around certain areas in NY, FL, AZ or NV they ask you to go to a specific site location which shows up along the request a card link.
