Is it possible to start the visa work permit process without a job offer? in Italy UK or wherever?

As for Italy: a visa work permit I'm not really sure; anyway a permit, yes. If you can prove you can financially keep yourself, you may even apply for elective residence without a work.

a work visa is given if you have a job offer only, the employer is the one who will do the application , in the country where the job is offered ...not you .......

the fact that you are a US citizen does not make it harder ,on the contrary ,if you are looking for a particular field , look on the Internet for job offers in your field and the country chosen by you .
