  1. For India you need a Visa to enter the United States but you need a passport to get a Visa.  Understand?  Once you are here and get your passport stamped upon arrival you will have an expiration time for your stay but with that stamp you can travel anywhere you want in the United States.  Understand?

  2. you need nothing to travel between states in the US

  3. You need a passport, and maybe a visa, to enter the US, but to go from state to state, you don't need anything. There aren't even any check points or anything like that, just a sign saying "Welcome to (our state)".

  4. No, you don't need a visa to travel between states.

  5. One does not need a visa to travel between states in the USA.

    Regarding your second question:

A passport is issued by the country of which one is a citizen.  This document certifies who you are.

A visa is issued by the country that one is visiting, granting one permission to visit that country.  Some countries also require an exit visa, which gives one permission to leave that country.
