  3. no, but some local rules may dictate trees in neighborhoods, but not in forest own by the government.

  4. I doubt it, but it makes sense. Diseased trees transfer diseases.

    Dutch Elm comes to mind. I've seen Coconut Palms get diseased. Trees pass diseases in stands. If one is infected, best to cut it before it infects all the others.

    For your question, no. No law dictates a diseased tree must be removed from a forest, in the US. I'm not sure of other countries , or continents.

  5. No.  That is why tree-huggers and loggers are at "loggerheads".  The loggers want to remove the diseased and damaged trees, that have some economic value, and the tree-huggers want to keep them around to decay, decompose, burn, and otherwise add to the ecological ambiance of the forest.
