  1. Neither, but indirectly by the people. It's a parliamentary system like Britain. The Prime Minister of Canada is the leader of the political party that wins the majority of seats in Parliament.

    The Prime Minister is the leader of whichever party gains the most seats in a federal election.

    The Queen is the head of goverment, but its purely titular.  Her representative is the Governor-General, an appointed post, again with only ceremonial powers.
