If you are talking about a modem from your ISP, you do not need an adapter, there is one RJ11 connection, that is for the phone line, and (depending on model) there are one or more RJ45 ports that connect to a router or a PC with a cat5 (Ethernet) cable.  If you are talking about a dial up modem, (56kb or V.90 etc) then no, there is not adapter.
