all of those answers are good... BUT none really addressed your question in reference to a calling card...

try a spoof card.. it will let you choose what numbers show up on the CID

Many VoIP services allow you to buy phone numbers (DIDs) in any USA/Canada area code.

Some examples are:

InPhonex - http://www.inphonex.com/services/voip-li...

CallCentric - http://www.callcentric.com/dids/pay_per_...

InPhonex also has a Calling Card service feature that may be able to show your DID phone number as the calling number.

Also, if you have Skype's SkypeIN service, as part of SkypePro, I believe you may be able to set your SkypeOut caller ID as your SkypeIN number.

Skype offers numbers in many USA area codes.
