  2. USP stands for United States Pharmacopoeia. It is a large very boring book of standards. For instance the standard for commercially available drug X is at least 92% pure with less than X amount of contaminate Y and the tablets will dissolve during a certine test in z seconds.

    To answer your question there is no difference in the States but other countries are free to do as they wish.
