  1. There are many causes for forgetfulness. Alzheimer's is considered an old person disease but it can effect younger people. Autoimmune system diseases cause short term memory loss and the list goes on.  from brain tumours to just plain old tiredness, to lack of paying attention, to stress. You really need to see a medical practitioner to have this checked out if you feel it is a real problem. Keeping the brain active with puzzles and crosswords will help but any sinister reason must be ruled out.

  2. Depends on your age. Alzheimer's if you're old.

  3. Sounds like dementia.

Read books or do crosswords or suduko.  Your brain is a muscle.  You have to use it constantly if you want it to stay sharp.

If you think it is dementia, you'll see your doctor?
