Is there a downloadable template for an NFL betting sheet for easy print out and distribution somewhere?

I'm trying to do an office pool, where it shows the teams and the spreads and has a ripoff at the bottom for betting purposes. I can't seem to find a template anywhere for this. Anyone know of one and can link me to it? Thanks!
