The countries I have visited "keep to the left" with their trains because, I suppose, their first railway systems were designed by the British.

  1. It varies.  For instance most railway operations in the USA tend to "run right".

For instance the Chicago Northwestern.  Most of their lines went NORTH-WEST out of Chicago, so they ran "keep to the left".  Why? Because commuters stood around the stations waiting for inbound trains in the mornings, but not in the afternoons (they just immediately went home).  Due to the prevailing wind around Chicago, the stations sheltered the waiting passengers better if they were on the north/east side of the tracks.  Trains served those stations best if they ran left-hand running, so they did!

  2. Possibly but it isnt always true.

There are things as single lines and bi directional lines.

  3. The United States, except for single track operations.
