Just left Cancun last week.  There is no exit fee.

Yes, but most often the price is included in your plane ticket.

If your driving you pay on the American side (toll both) going in (about $1.00 if I remember right), and you will then pay $21.00/person at any bank in Mexico. In addition, you will pay $2.00 at the toll boths on the Mexican side of the border when leaving Mexico.
