Yes, there was an incident in 2004 in which a man with cerebral palsy fell out of Superman: Ride of Steel at SFNE.  However, this was not an issue of the ride operating incorrectly, but rather a fault committed by the employees-- the man should not have been allowed to ride, as he could not sit properly in the ride's seat.

Since that incident, Six Flags added over-the-shoulder restraints to the ride (though they weren't necessary--but people were having fits about it).

Don't let this make you feel any less safe!  There's nothing wrong with the ride!
