  1. You have a double negative problem. "Doesn't" modifies the verb "know" and the two clauses are joined with a negative conjunction "nor."

    To check it, take out the negatives.

    "She knows that he is in Canada or his name."

    Corrected Sentence:

    "She doesn't know that he is in Canada nor does she know his name."

    Actually, this is just a really weird sentence structure and meaning. I'd probably restructure it to the following.

    "She doesn't know his name nor that he is in Canada."

    Should be -- She doesn't know his name OR (not NOR) that he is in Canada.

  3. Honestly, I am good at grammar but can not detect any sort of error. However the sentence doesn't sound right. I would say, "She doesn't know his name, nor that he is in Canada." It just flows better.
