  1. No.  All elective procedures will have a copay.


  2. It depends upon the benefits on your policy.

    If your insurance policy covers 100% of surgery charges in general, then it may cover 100% of a lap band surgery.  (Provided that there's no exclusion and you meet the medical criteria.)

    But, if your policy's benefit level is 90% for surgery, then you'd have to pay 10%.  Or 80%-20%, etc.

    So, to answer your question, sure...its possible that someone out there has 100% coverage for a lap band surgery.  If they are on a policy that covers surgery 100% with no deductible.  And their policy doesn't exclude bariatric surgery.  And they get medical prior approval for the procedure.

  3. No.  Health insurance rarely covers 100% of anything.
