what do you mean?, you are already a pilot once you acquire the license, but you probably meant an airline pilot, well unfortunately its true, many of us dont reach to the airlines even with the CL for many reasons;  time, money, health  marriage, kids etc, etc, etc its like a bet in the lottery you dont know what can happen, it takes a lot of commitment to get there.

Lots of people who hold Commercial Airman Certificates are not able to secure employment as pilots, for a wide variety of reasons, if that's what you are asking.

There is never any guarantee of employment, no matter what certifications and ratings you hold.

Airline pilot?  If so, then absolutely.  These days, I'd say a huge percentage don't make it to the major airlines.  There are also those who don't make it to the regionals.  Sometimes it's the pay, other times it's because of family obligations.  Lots of things.  Having a cpl is just the first step in a long road to a successful flying career.

If one becomes medically unfit or renders him/her self incapable of piloting an aircraft will not be able to get a job flying an aircraft.
