Your jaw can get tired, you can gag, you can just get bored while you're doing it.

You can wind up with some sticky stuff in your mouth, or perhaps in your eye.  If it gets in your eye, it's possible it may sting, and if you're blowing someone who has herpes, you could get herpes infection of the eye.

More generally, if you givie an infected person a bj, you can get an STD.
