  1. Sure. You will need to write the county you were born in.

You can look up how to do it online. I've done it a couple of times.

  2. Go to the hospital where you were born and ask them to check their records.

  3. Depending on where you were born ... I was born  in Fort Lauderdale, FL.

I had to go to the Bureau of Vital Statistics. It cost me about $15.

You may be able to go to your County's Government website to see if you can fill out a form on line.

  4. You get it from the county you were born in, NOT the hospital. The hospital certificate is not a legal document. Only the one issued by the county with the county seal on it (it's a raised seal) is legal. They cost about $5 a copy, and you don't even have to GO to the county, you can get it by mail. I got copies of mine by mail since I live in FL and was born in NJ. They just ask you to identify a couple facts, such as father's name, mother's maiden name, and date, and place of birth.

  5. No.  Not unless you are 18 and have ID.  Depending on the state you were born in, you either have to go to the county clerk of courts or state dept of HHS.

Either way, Call the Clerk of Court in the county you were born in.  They can tellyou who to contact and what info and ID you will need to provide.

  6. I'm pretty sure if you call the hospital that you were born at they can send you another copy.

  7. contact the local gov office were you were born, they can tell you what you need to do to get a copy of it.

  8. Try here: http://www.vitalchek.com

  9. You have to go to the city hall in the city you were born in.

  10. Call or write to the Department of Vital Statistics in the city where your were born.  It usually cost a few dollars. I've had to do this for my husband, our kids and myself because we lost ours in a fire.

If you are filing for welfare, your counselor/case worker can  order one for free.

The hospital you were born at will not have this information if you were born in the US.
