    Unofficially, it's when someone has had a bowel movement and needs to be cleaned up.

  4. Never heart that (means probably that a patient has soiled him or herself.  Which may happen with paralysis (traumatic injury to the spine), parasympathetic responses (grave bodily stress), or other conditions.  It's not a nice way to address the cleaning the patient needs :(

  6. Expert Realtor is correct. It's just a nursing slang thing for when a patient has a large bowel movement and needs cleaning up.
